Good Faith Estimate (No Surprises Act)

You are entitled to receive this estimate in writing before your scheduled service. This estimate outlines the cost of the care you are likely to receive so there are no financial surprises. If your actual bill is significantly higher than the estimate, you may have the right to dispute the charges. Please contact our office to request a Good Faith Estimate or for more information.
